Program Overview
Helping Medicaid Members Access Available Benefits and Community Supports
The Health Equity Alliance of Long Island (HEALI) program helps eligible Medicaid members connect with resources and support services that may be available through their health plan benefits.
Many individuals are unaware of the additional services for which they may qualify. HEALI works to bridge that gap by providing guidance, education, and personalized assistance to help people better understand and access available benefits that support their health, stability, and overall well-being.

The Reason
Why HEALI Matters
Health is influenced by much more than medical care alone. Factors such as stable housing, access to nutritious food, reliable transportation, and social support all play a critical role in an individual’s overall health and quality of life.
The HEALI program helps address these social factors by connecting eligible Medicaid members with resources designed to support their daily needs and promote healthier outcomes.
Through education, advocacy, and personalized support, our team helps participants make the most of the benefits available to them.
The Solution
How HEALI Can Help
Depending on individual eligibility and health plan benefits, HEALI may assist participants in accessing services such as:
- Food and nutrition support
- Transportation assistance
- Housing-related support services
- Care coordination and navigation
- Community resources and referrals
- Other enhanced benefits available through Medicaid plans
Our team works one-on-one with participants to help identify available resources and simplify the process of connecting to those services.
The Implementation
Our Approach
HEALI’s approach is simple:
Identify
We help participants understand what services and benefits may be available through their Medicaid coverage.
Connect
We assist individuals in navigating systems and accessing appropriate resources and support services.
Support
We provide ongoing guidance and referrals to help participants connect with services that can positively impact their health and well-being.
Program Impact
The HEALI program is dedicated to helping Medicaid members gain greater awareness of available supports and reducing barriers to accessing critical resources.
By connecting eligible individuals with available benefits and community-based services, HEALI helps strengthen health outcomes, improve stability, and support healthier communities throughout Long Island.
